Restored the ability to do a refund to a credit card by manually entering the card number but added a confirmation dialog to warn of possible scam.

Added a textbox to reports allowing freehand entry of Code/Fine/SubFine values for items in addition to the current dropdown boxes (e.g. Item Listing report).
